You need a Kentucky A06 Permit when your vehicle or load exceeds Kentucky legal size and/or weight limits for a specific movement that does not qualify under standard annual permit categories like the A02 non-divisible permit.
You need an A06 permit when your load exceeds one or more of these Kentucky legal limits:

Height over 13'6"

Gross weight over 80,000 lbs
